Schistosomiasis is an important cause of disease in many parts of the world, most commonly in places with poor sanitation. School-age children who live in these areas are often most at risk because they tend to spend time swimming or bathing in water containing infectious cercariae.

Amebic liver abscess and sympathetic pleural effusion
Approximately 20% to 35% of patients with an amebic liver abscess will have a sympathetic pleural effusion
Pleuro-pulmonary penetration of amoebic liver abscess occurs in 15%-20% cases. It develops when a right lobe abscess penetrates the diaphragm and produces an empyema or broncho-pleural fistula. Such involvement is associated with right lower chest pain, usually accompanied by persistant cough. When an abscess penetrates a bronchus, expectorated material has the characteristics of amoebic pus.
Examination of the pleural fluid from patients with subphrenic abscesses usually reveals an exudate with predominantly polymorphonuclear leukocytes.
Although the pleural fluid WBC may approach or even exceed 50,000/mm3, the pleural fluid pH and glucose level remain above 7.20 and 60 mg/dL, respectively. It is distinctly uncommon for the pleural
fluid to become infected (26). However, empyemas have resulted from contamination of the pleural space when the abscesses were drained percutaneously

Diagnosis of systemic bacterial infection/sepsis
SIRS, sepsis, severe sepsis, and septic shock were categorised according to the criteria of the consensus conference of the American College of Chest Physicians/Society of Critical Care Medicine.
PCT <0.5 ng/ml
Systemic infection (sepsis) is not likely
Local bacterial infection is possible
Low risk for progression to severe systemic infection (severe sepsis)
Caution: PCT levels below 0.5 ng/ml do not exclude an infection, because localised infections (without systemic signs) may be associated with such low levels. Also if the PCT measurement is done very early after following bacterial challenge (usually 0.5 and 2 and 10 ng/ml
Important systemic inflammatory inflammatory response, almost exclusively due to severe bacterial sepsis or septic shock
High likelihood of severe sepsis or septic shock.
The optimal cut-off values ranges of Procalcitonin (PCT) are variable and dependent on
1.the clinical setting (eg, emergency room, ICU, post-operative or trauma patients)
2.the site and extent of the infection (eg, RTI, meningitis, abdominal infection)
3.co-morbidities (eg, immunosuppression)
4.the clinical implications drawn (eg diagnosis, prognosis, antibiotic stewardship).
Increased PCT levels may not always be related to systemic bacterial infection.
There are a few situations described where PCT can be elevated by non-bacterial causes. These include, but are not limited to
1. neonates 48 hrs PCT สูงได้ ไม่เกิน 10ng/ml แต่ถ้า < 2ng/ml exclusion of sipsis (NPV 94%)
3.PCT in autoimmune dis cut-off <1.0 ng/ml
Low PCT levels not always indicate absence of bacterial infection.
Falsely low PCT levels in the presence of bacterial infection may accour eg. in case of
1.early course of infections
2.localised infections (see chapter PCT & Infection)
3.subacute infectious endocarditis.

ถ้าคะแนนทั้งวันที่1และวันที่3 จากการประเมินยังได้=หรือ 6 จึงให้ยาปฏิชีวนะต่อตาม การรักษาปอดอักเสบ ซึ่งสามารถนํามาประยุกต์ใช้ทางคลินิกเพื่อช่วยประกอบการพิจารณาลดระยะเวลาการให้ยา ปฏิชีวนะนานเกินจําเป็นลง (คําแนะนํา IA)
ถ้าตรวจไม่พบแบคทีเรียหรือเซลล์ของการอักเสบโดยไม่เคย ได้รบั หรอื เปลี่ยนยาปฏิชวี นะภายใน 72 ชัว่ โมง โอกาสที่ผู้ป่วยจะมี HAP หรือ VAP จะน้อยมาก จงึ ควรหาสาเหตอุ ่ืน ของการมีinfiltrationจากภาพถ่ายรังสีทรวงอกไข้ หรืออาการหอบเหนื่อยที่ไม่ใช่การติดเชื้อแบคทีเรียต่อไป
